Introduction

The story of Ignaz Semmelweis is one of the clearest historical examples of why an appeal to tradition can become dangerous. In the 1840s, doctors in leading European hospitals followed accepted routines that were considered normal, respectable and scientific. Yet one of those routines—moving directly from autopsies to examining women in maternity wards without proper hand disinfection—was helping to spread a deadly disease. When Semmelweis presented evidence that a simple handwashing procedure dramatically reduced deaths, many physicians resisted the change. The case shows that long-standing professional habits can persist even when evidence points in the opposite direction, and it illustrates why tradition should be treated as a hypothesis to test rather than proof that a practice is correct. When Medical Tradition Cost Lives

The accepted routines Semmelweis challenged

In the mid-nineteenth century, puerperal fever, also called childbed fever, was one of the most feared causes of death after childbirth. At the Vienna General Hospital, where Semmelweis worked, mortality rates differed dramatically between two maternity divisions. The division staffed by doctors and medical students consistently experienced far higher death rates than the division staffed largely by midwives. Women were so aware of the danger that some reportedly begged not to be admitted to the doctors’ clinic. The crucial difference was hidden within ordinary professional practice. Medical students and physicians routinely performed autopsies and then examined pregnant women. Hand disinfection was not a standard expectation. The prevailing medical culture did not view doctors themselves as carriers of disease, and many physicians moved between the dissecting room and the maternity ward as part of their normal work. Semmelweis became convinced that material transferred from corpses was infecting patients. After introducing mandatory handwashing with a chlorinated lime solution in 1847, mortality rates fell dramatically. Different historical analyses report somewhat different figures, but all agree that deaths dropped from previously high levels to a small fraction of their former rate, providing unusually strong practical evidence for a medical intervention. What makes this episode especially relevant to the appeal-to-tradition fallacy is that Semmelweis was not proposing an elaborate new technology. He was challenging a familiar routine. The established practice felt normal because it had become embedded in medical training and professional culture. Its age and acceptance gave it an authority that exceeded its evidential support.

Why Evidence Met Professional Resistance

The resistance to Semmelweis was not simply a matter of stubbornness. Several forces combined to protect existing practice.

First, his findings conflicted with dominant medical theories. Germ theory had not yet been established, so Semmelweis could demonstrate that handwashing worked without being able to provide the biological explanation that later generations would recognise as correct. To many contemporaries, the proposed mechanism seemed speculative or implausible. Second, his conclusion carried an uncomfortable implication: doctors themselves were helping to kill patients. Accepting his evidence required respected professionals to acknowledge that their ordinary behaviour was contributing to maternal deaths. That challenged professional identity as well as medical theory. Third, established routines often enjoy a presumption of correctness. If a practice has been taught by senior physicians, repeated by generations of practitioners and embedded in institutional life, abandoning it can feel risky. The familiar procedure acquires authority simply because it is familiar. In logical terms, this is where an appeal to tradition can emerge: the fact that a practice is longstanding begins to substitute for evidence that it is effective. [The James Lind Library]jameslindlibrary.orgThe alarmingly high mortality in the first clinic had defied explanation until Semmelweis was…Read more…

Some historians also note that communication problems, professional rivalries and disputes over the interpretation of evidence contributed to the controversy. Resistance was therefore not caused by a single factor. Nevertheless, the persistence of accepted routines despite striking mortality data remains one of the most memorable features of the case. What the Case Teaches About Tradition and Proof

The Semmelweis episode does not show that traditions are always wrong. Medical traditions often preserve valuable experience and practical knowledge. The lesson is narrower and more important: a practice should not be accepted merely because it is established.

Several insights emerge from the case:

  • Tradition can contain hidden errors. The doctors involved were not acting maliciously. They were following accepted procedures that had become normal through repetition. [PMC]pmc.ncbi.nlm.nih.govPMCThe little-known history of cleanliness and the forgottenPMCby P Poczai · 2022 · Cited by 30 — Doctors did not routinely wash their hands until the mid-1800s, and they would proceed straight fro…
  • Evidence can arrive before theory. Semmelweis observed a powerful reduction in mortality before medicine possessed a complete explanation for why the intervention worked. [PMC]pmc.ncbi.nlm.nih.govAs soon as he started working in this ward, Semmelweis was struck by the difference […Read more…
  • Professional consensus is not infallible. A widely shared belief may reflect institutional habits as much as empirical proof. [Science History Institute]sciencehistory.orgignaz semmelweisScience History InstituteIgnaz Semmelweis15 Sept 2025 — He realized that physicians were spreading infections and that handwashing could…
  • Changing routine can be psychologically difficult. Evidence sometimes demands that experts revise assumptions about their own behaviour, making acceptance slower than the data alone would predict. In the context of logical fallacies, Semmelweis’s story demonstrates the weakness of reasoning that relies on familiarity as justification. The old routine seemed trustworthy because it was customary, not because it had been rigorously validated. Once systematic observation and comparative evidence were brought to bear, the tradition could no longer defend itself on age alone. A Lasting Example of Medicine Correcting Itself

Today, hand hygiene is regarded as a foundational element of infection control and patient safety. Modern healthcare systems treat handwashing as an evidence-based practice rather than a matter of custom. The significance of Semmelweis’s work therefore extends beyond obstetrics. It illustrates a broader principle of scientific and medical progress: inherited practices deserve investigation and respect, but they must remain open to testing and revision. For discussions of the appeal to tradition, few examples are more vivid. Lives were lost not because evidence was unavailable, but because established routines enjoyed the protective force of habit, authority and familiarity. Semmelweis’s challenge reminds us that the age of a practice is not proof of its correctness; evidence must always have the final word.
